FOND DU LAC – SSM Health is investing $30,000 in the Fond du Lac Family YMCA to expand community-based wellness programs focused on chronic disease management, recovery and healthy lifestyles.
The funding is expected to support programs serving about 556 participants in 2026, including LIVESTRONG at the YMCA for cancer survivors, the Healthy Living Program, Parkinson’s Exercise Program, Healthy Hearts and Balanced Health educational seminars.
SSM Health Community Health Manager Laura Nakielski said the investment helps extend health care beyond clinical settings by connecting residents with preventive care, recovery services and ongoing wellness support.
“At SSM Health, we recognize that health is shaped not only by clinical care but also by the support people receive in their communities,” she said. “By investing in these programs, we are strengthening access to preventive care, chronic disease management and recovery services that improve quality of life for people across our community.”
Nakielski said the partnership between SSM Health and the YMCA spans more than 15 years.
In 2025, she said the YMCA’s Healthy Living Program received 132 referrals, including 112 from SSM Health providers.
