FOND DU LAC – The Agnesian HealthCare Foundation, in partnership with the Fondy Food Pantry, has donated $5,000 to support food-insecure patients at SSM Health St. Agnes Hospital.
The funding – per the release – will provide food bags for patients identified as food insecure during intake, helping ensure access to nutritious food after hospital discharge.
“This grant reflects the Agnesian HealthCare Foundation’s commitment to meeting patients where they are and supporting programs that have a meaningful, immediate impact,” Philanthropy Director Mason Gravelle said.
Laura Nakielski, SSM Health community health specialist, said the initiative is part of a program that screens Medicare and Medicaid patients for food insecurity and connects them with community resources.
Launched as a pilot in 2024, Nakielski said the effort currently serves an estimated 80 to 110 patients annually.
“This generous support allows us to provide immediate assistance while also connecting patients to longer-term resources,” she said.
Nakielski said the program supports broader efforts to address social determinants of health and improve long-term patient outcomes.
